1.  Appointment

- Book 30 minute appointment with Doctor Lawson

- appointment cost $220 with $85 rebate from medicare ($135 out of pocket cost)

2.  Supportive evidence

- obtain supportive evidence that confirms ADHD diagnosis

- evidence can be from a doctor, psychiatrist or psychologist

- email evidence to Noosaville.mc@parteneredhealth.com.au or bring to initial appointment

3. Follow up appointments

  - any follow up appointment billed as per standard 15 minute appointment

-  appointment cost $105 with $44 rebate form medicare ($60 out of pocket cost)
